This is to help you know what to expect in case of filing an appeal in regards to your assessment. I am providing links to information provided by the State.
 
This page at the Indiana Department of Local Government Finance (DLGF) provides a general overview. Including a link to the optional state form 130, for beginning an appeal on a subjective basis. Form 130 is similar to the pdf requestin that neither is required, but may be used for convieniece, as the information requested on each IS required.
 
The information can be provided in just about any legible form, as long as it includes the taxpayer's name, the address and key number of the property in question, and the address and phone number of the taxpayer. A email adress can be provided as well, but is also optional.
 
Please note that while these forms are linked for convienience, and E-mail is a convienient tool, combining the two can be less so. The forms are great for physical delivery, but if you intend to file by email, it may be easier to simply use them as a guide, rather than fill them out, scan them back in and send them as attachments.
  
This fact sheet on the trending process can also provide useful information. For instance it points out the lag in values used for annual adjustments. If you are appealing on current market value of your home, keep in mind that there is a lag between assessed value and current market. As the real estate market stabilizes or recovers values will either match up, or potentially show as being in your favor.
